The SMAS is the layer of connective fibrous tissue located between the facial muscles and the skin. During a SMAS facelift, the SMAS layer is pulled and stretched using sutures to adjust the contours of the face. 

Sometimes, this can result in the unnatural or “overdone” appearance that many facelift patients fear. During a deep plane facelift, the surgeon carefully elevates the SMAS layer from the facial muscles beneath it. Additionally, the ligaments that keep the SMAS layer anchored are released. This allows the doctor to lift the SMAS without tension, which creates deeper, more natural-looking results.

Once the deeper tissues have been adjusted, the excess skin is excised, and the skin is closed.
