A surprised look after surgery has more to do with brow elevation, or a brow lift, than it does with a facelift. Facelift is really a lower facelift that involves mostly a neck lift and then the cheek lift. So a surprised look is really impossible after a regular lower facelift. Brow lifts can have a surprised look if the physician elevates the eyebrows too high, and this is why you need to go to an experienced plastic surgeon that performs brow lifts.
